We provide resource navigation, benefits and forms assistance, SSI/SSDI application support, care coordination, and monthly navigation support for individuals and families in Sacramento County and surrounding communities.

Making a referral
How the referral process works.
Referrals are straightforward. Here is what to expect from first contact through to follow-up.
Obtain client consent
Before sharing any client details, confirm the individual has given permission for their information to be passed to BCSG.
Send the referral
Contact BCSG by phone or email, or use the agency enquiry form below. Include the client's name, contact details, and the type of support needed.
Initial contact with the client
BCSG will reach out to the referred individual to introduce the service, confirm their needs, and discuss next steps.
Follow-up with your organisation
Where agreed, BCSG will provide a follow-up update to the referring organisation. Communication expectations are confirmed at the time of referral.
What to include in a referral
- Client's full name and preferred contact method (phone or email)
- Type of support needed (e.g. housing navigation, SSI/SSDI, benefits forms)
- Any relevant context that would help BCSG prepare for the first conversation
- Your organisation's name and a contact person for follow-up
- Confirmation that the client has consented to the referral
